Under direct supervision, assists in preparing for and offering direct individual and community services, client advocacy, and providing both general and specific health interventions in the community, clinic and home settings. This position is primarily responsible for performing TB disease and infection case investigations and surveillance. Work involves the ability to adhere to the Rules and Regulations for the Control of TB issued by the Illinois Department of Public Health (IDPH), Center for Disease Control (CDC) and guidance provided by the TB clinic Medical Director and TB Program Coordinator. This position demands independent judgment and discretion in making position-related decisions. This position performs administrative functions and other database related duties pertaining to the TB Program as assigned.
- Proof of COVID-19 vaccination or medical/religious exemption within 30 days of hire. If not fully vaccinated, you must follow the requirements for unvaccinated until you meet the definition of fully vaccinated.
Scheduled Hours: 37.5 hours a week
- Monday through Friday – (9:00am – 5:00pm)
