Summary
An outgoing and empathetic community advocate, the COVID-19 Community Health Worker (CHW) conducts direct outreach and education for community members at various locations, including community centers, public gathering places, and individuals’ homes. The COVID-19 Community Health Worker program is an arm of the Pandemic Health Navigator Program (PHNP), a grant-funded, short-term program focused on reducing the disparities of outcomes of COVID-19 seen in our community through education/outreach, resource coordination, and contact tracing support. The CHW assesses and addresses community member needs related to primary healthcare and social determinants of health, provides basic health education about COVID-19 and chronic conditions, and supports individuals in accessing vaccination opportunities. Where needed, CHWs work with healthcare and social service providers to facilitate resource/care coordination for community members who have complex medical and social needs. The CHW may also refer cases requiring additional support to the Community Health Worker (CHW) Lead and/or COVID-19 Case Managers.  
 
 This is a field-based position. The PHNP is grant-funded through December 31, 2021.
